- Photo ID
- A list of current medications
Please do not wear any perfume,colgne or scented lotion as these may trigger allergies or migraines for people who are susceptible.

Your copay, co-insurance and/or deductible is due at the time of service. We are happy to accept self-pay patients. For Self-Pay patients we offer a payment plan. We accept cash, check and most major credit cards.
